Chornak (the "Petitioner" or "Mother"), by and through her attorneys, Wix, Wenger & Weidner, and files this Petition for Name Change on behalf of Petitioner's daughter, Kathryn Grace Mroczkowski, stating as follows: 1. Petitioner is Jennifer E, Chornak, an adult individual who resides at 51 White Oak Boulevard, Mechanicsburg, Pennsylvania 17050, 2. Petitioner is the natural Mother of Kathryn Grace Mroczkowski, born May 14, 1998 (age 7) (the "Minor Child"). 3. The natural father of the Minor Child is Douglas R. Hughes (the "Father"), an adult individual who resides at 244 Old Leacock Road, Gordonville, Pennsylvania 17529. 4, Petitioner has joint legal and primary physical custody of the Minor Child by agreement between Mother and Father. 5, The residences of Petitioner and of the Minor Child over the past five years are as follows: a. 51 White Oak Boulevard, Mechanicsburg, PA 17050 - from April 2004 to present; b, 1125 Columbus Avenue, #3, Lemoyne, PA 17043 - from May 1998 to March 2004. 6. Pursuant to 54 Pa,C.S, 9701, Petitioner files this Petition for Name Change, intending to change the name of the Minor Child from Kathryn Grace Mroczkowski to Kathryn Grace Chornak, 7, The Minor Child was given the Petitioner's maiden surname (Mroczkowski) at birth, 8. On April 24, 2004, Petitioner married Michael Chornak and took Chornak as her surname, 9. Petitioner desires to change the name of the Minor Child so the Minor Child will have the same surname as her Mother and her step-father. 10. Granting this Petition for Name Change is in the Minor Child's best interest for the following reasons: a, The Minor Child's current surname is different than that of her natural Mother and of her natural Father; b. The Minor Child is now of school age and the Petitioner desires to protect the Minor Child from the psychological and emotional confusion that may result from the Minor Child and her parents all having different surnames. 11, To the best of Petitioners knowledge, there are no judgments or decrees of record or any other matter of like character entered against the Minor Child. 12, The Father consents to the change of name requested in this Petition as evidenced by the signed and notarized Consent, attached hereto as Exhibit A and incorporated by reference. WHEREFORE, Petitioner respectfully requests that this Honorable Court: a. Schedule a hearing on this Petition; b.
