PURSUANT TO 54 Pa.C.S.A. SECTION 701 ET SEQ.
AND NOW, comes Travis Lamaar Jones, by and through O'Brien, Baric and Scherer,
Attorneys at Law and makes the following Petition:
1. Petitioner is Travis Lamaar Jones by his father and natural guardian, Stanley C.
Kendrick. Petitioner is a minor individual residing at 3234 Ritner Highway, Carlisle,
Cumberland County, Pennsylvania 17013.
2. Petitioner was born March 11, 1989 in Carlisle, Pennsylvania.
3. At the time of birth, Petitioner was given the name Travis Lamaar Jones. A copy
of the birth certificate evidencing this fact is attached hereto and incorporated herein as
"Exhibit A"
4. Petitioner has resided continuously in Cumberland County since his birth,
5. There are no judgments or decrees of record against Petitioner.
6. Petitioner now desires to have his name changed to Travis Lamaar Kendrick, so
that his surname is the same as that of his father.

WHEREFORE, Petitioner requests your Honorable Court to enter an Order granting the
Petitioner's request to change his name from Travis Lamaar Jones to Travis Lamaar Kendrick.

ORDER OF COURT
AND NOW, this 22nd day of June, 2005, upon consideration of the attached
Petition To Change Name Pursuant to 54 Pa. C.S.A. S,'ction 701 et seq., a hearing is
scheduled for Monday, August 29, 2005, at 1 :30 p.m., in Courtroom No.1, Cumberland
County Courthouse, Carlisle, Pennsylvania.
IT IS FURTHER ORDERED that notice of the filing of said Petition and the
aforesaid date, time and place of the hearing be published once in the Cumberland Law
Journal and a newspaper of general circulation in the County at least 10 days prior to the
hearing; in addition, Petitioner shall comply with all oth<~r requirements of C.C.R.P. 5,
and the Act of December 16, 1982, P.L. 1309, ~6 (see 54 Pa. C.S. ~70I [Historical and
Statutory Notes] [Main Volume]), and 54 Pa. C.S. ~~70I et seq.
IN ADDITION, notice of the petition and hearing shall be provided to the minor's
mother at least 10 days prior to the hearing, to the extent that her whereabouts are known.
