Birth Date and Place: Petitioner was born on March 20, 1966, in Tianjin, China, A copy of Petitioner's birth certificate is attached hereto and marked as Petitioner's Exhibit "A". 2. Birth Name: Petitioner was named Ding Xiaoxia at birth, and has been known by that name with the following exception: A, On November 26, 2004, Petitioner married Jeffery C. Edmonds in Mechanicsburg, Cumberland County, Pennsylvania, (See attached and marked as Petitioner's Exhibit "B") B. At the time the marriage application was taken, the Clerk of the Orphans Court of Cumberland County, Pennsylvania, agreed to list Petitioner's name in "translation" from her Mandarin Chinese spelling of Ding Xiaoxia to Sara Lynn Ding. (See attached and marked as Petitioner's Exhibit "C") 3. A On April 14, 2005, Petitioner applied for a Social Security Number Card whereby the Social Security Administration established her Social Security number to be 188-82-7117, (See attached Application marked as Petitioner's Exhibit "D" AND also see copy of Petitioner's Social Security Card attached and marked as Petitioner's Exhibit "E") B. Please note that the Social Security Administration confirmed that her name at birth was Xiaoxia Ding. Further, they acknowledged the Marriage Certificate and listed her Applicant name as Sara L. Edmonds Ding, 4. Petitioner was issued a passport from the People's Republic of China on January 21, 2002, whereby her name was listed as Ding Xiaoxia. (See attached and marked as Petitioner's Exhibit "F") 5. Petitioner applied for a U,S, Department of Homeland Security Employment Authorization Card on or about February 23, 2005, whereby said authorization was granted and card number MSC0509519813 was assigned in the name of Sara L. Edmonds Ding. (See attached and marked as Petitioner's Exhibit "G") 6. Residence: Petitioner resides at 137 Appalachian Drive, Carlisle, Cumberland County, Pennsylvania, 17013, and has lived at this address since October of 2004. 7. Petitioner's Parents: The father of the Petitioner is named Ding Qianchang, and he resides in Tianjin China. The mother of the Petitioner is named Li Shuqin, and she resides in Tianjin, China. (See attached and previously marked as Petitioner's Exhibit "A") 8. Reason for Name ChanQe: Petitioner seeks to change her name back to her Mandarin Chinese spelling of Ding Xiaoxia Sara Lynn Edmonds and offers the following reason for seeking to do so: A Due to the confiicting spellings and enunciations of her Birth Certificate; Marriage Application; Passport, and other documents, the U,S. Department of Immigration has denied her request to have her fourteen (14) year old daughter, Hang Mingqi, the authority to enter the United States for purposes of being reunited with the Petitioner, her birth mother: (See attached and marked as Petitioner's Exhibit "H") B. Please note that the Social Security Administration confirmed that her name at birth was Xiaoxia Ding. Further, they acknowledged the Marriage Certificate and listed her Applicant name as Sara L.
