Petitioner, Sarah Jennings Hermeling, is an adult individual residing at 225 South West Street, Carlisle, Cumberland County, Pennsylvania. 2. Petitioner currently has her father's last name, and after her paren,ts divorced years ago, petitioner has had no relationship with her natural father, and as such, Petitioner wishes to cut ties with her natural father. Petitioner does not wish to change her name for any illegal purpose or to avoid any obligation(s). Petitioner has been fingerprinted by the Pennsylvania State Police and her fingerprint card is attached hereto as Exhibit" A" and by reference incorporated herein. 10. Petitioner has never been convicted of a felony. 11. Petitioner's Social Security number is 236-31-6369.
