Services were rendered and charged pursuant to applicable rate resolutions of the Mount Holly
Springs Borough Authority, for whom Claimant is administrative agent.
Attached hereto is a statement of the kind and character of the services so rendered and

with the Claimant, which sum of$591.91 for the above period was duly assessed against the
above Owner and premises beginning December 21, 2000 and for which sum, which includes.
