For sewer services rendered from 04/01/00 to 07/01/01.5aid services were rendered
pursuant to ordinance. Attached hereto is o statement of the kind and character of the
services so rendered and the price charged therefor, said statement being a true copy of
the owner's account with Hampden Township which sum of $192.63 was duly assessed
against the hereinabove-named owner and the premises on 07/01/0! and for which sum, with
interest, penalties and costs, a lien is claimed against the above-described premises in
accordance with the Act of May 16, 1923, P.L 207 (53 P.5. $ection 7101, et seq.), os
amended and supplemented, and the Act of May 2, ]945, P.L 382 (53 P.S. 5action 301,
et seq), as amended and supplemented.
