PETITION FOR CHANGE OF NAME
AND NOW comes petitioner, Angela Elaine Myers, by her
attorney, Kent H. Patterson and files this petition for change
of nwme as follows:
1. Your petitioner is Angela E. Myers who resides at 13
Railroad Avenue, Borough of Shiremanstown, Cumberland County,
Pennsylvania (Shiremanstown, PA 17011).
2. Your petitioner is an adult individual, currently age
26, having been born October 18, 1977.
3. Your petitioner has had the following residences
during the past 5 years:
13 Railroad Avenue
Shiremanstown, PA 17011
37 Falcon Court
Mechanicsburg, PA 17055
4. Your petitioner desires to change her name from Angela
Elaine Myers to Angela Elaine Schlessman.
5. Petitioner wishes to change her last name to
Schlessman for the following reasons:
a. Petitioner's last name of Myers is the name of
her mother and father who are now divorced and her mother will
be resuming her prior maiden name of Schlessman.
b. petitioner had emotionally painful experiences
when growing up with her father and has shunned using the last
name of Myers.
c. petitioner would prefer to use the name of her
mother and maternal grandparents with whom she has a close
relationship.
6. There are no judgments or decrees of record or any
other matter of like character against petitioner in the Office
of the Prothonotary of Cumberland County or, to the best of
petitioner's knowledge, in any other county or place.

WHEREFORE, petitioner requests your honorable court to
schedule a hearing and to thereafter order that the name of
petitioner be changed from Angela Elaine Myers to Angela Elaine
Schlessman.
