August 9,2005 Register Of Wills Cumberland County Court House Carlisle, P A 17013 To Whom It May Concern: Please be advised that the information sent by Mid Penn Bank in regard to the ownership of Certificate of Deposit 309001192 was incorrect. The information provided stated that the certificate was held in joint ownership between Mary Jane Fisher, Edward E. Fisher and Samuel W. Fisher. Ownership for the certificate was changed on December 29, 2004. Edward E. Fisher, the decedent, was removed as an owner prior to his death and Samuel W. Fisher was added as an owner on that date. The changes were made to the certificate though the computer system was not updated to reflect the changes. However, there is an exclusion not to exceed $3,000 per transferee regardless of the value of the account or the number of accounts held.
