PETITION FOR THE APPOINTMENT OF A GUARDIAN OF TBE"PERSOl'DOF

AND NOW, Petitioner, Aeryelle D. Dubois, a minor over the age of fourteen, hereby
petitions this Honorable Court to hold a hearing to confirm the consent of the natural
!Dother, Dawn M. Garcia to guardianship of the above-named minor child and, in support
thereof, aver(s) as follows:
1. AERYELLE D. DUBOIS, age 17, the child who is the subject of these
proceedings, was born on 04/18/1988 who resides at 327 South Sporting Hill
Road, Cumberland County, Pennsylvania.
2. Dawn M. Garcia, is the natural mother of Petitioner, whose location after her
release from The Cumberland County frison, is 23 West Green Street,
Mechanicsburg, Pennsylvania 17055.
3. Nathan Joppy, is the natural father of Petitioner whose location is unknown and
has no contact with Petitioner.
4. Dawn M. Garcia, the natural mother of Petitioner consents to the Petition. The
original consent, executed on 05/05/05 is attached to this Petition as Exhibit A.
5. Nathan Joppy, the natural father of Petitioner does not consent to the Petition as
his location is unknown and he has no contact with Petitioner.
6. The appointment of a guardian is necessary because the minor child has no legal
guardian.
7. Petitioner requests this court to appoint as guardian, John and Belinda Nester,
husband and wife, ages 42 and 40 who reside at 327 South Sporting Hill Road,
Cumberland County, Pennsylvania. The proposed guardians have no relation to
the Petitioner and the consent of each is attached to this Petition as Exhibits B and
C.
8. The proposed guardians are over the age of 18 and have no interests that are
adverse to that of the minor, and are not fiduciaries or sureties, or employees of a
corporate fiduciary or surety, of an estate in which the minor has an interest.

9. The religious persuasions of the natural parents are not known. The religious
persuasion of the minor is Christian and the proposed guardians are Christian.
WHEREFORE, Petitioner, Aeryelle D. Dubois, requests this Honorable Court to hold
a hearing to confirm the consent to guardianship ofthe natural mother Dawn M. Garcia
of the above named minor child to John and Belinda Nester.
