Epperson and Albert Epperson, paternal grandparents, reside at 26 Cobblestone Drive, Carlisle, Pennsylvania. 2. Defendant Carmen Lancaster, natural mother, resides at 1003 South Center Street, Grove City, Pennslvania. 3. Plaintiffs seek clarification of custody of the following child: Name Present Residence Aae Alexis V. Epperson with Paternal Granolparents 7 The child was born out of wedlock. The child is presently in the custody of paternal grandparents. 4. Father currently resides with the following persons: Name Relationship Melissa Epperson Wife 5. Mother currently resides with the following persons: Name Relationship Not known Her stepfather 6. Plaintiffs have not participated as a party or witness, or in another capacity, in other litigation concerning the custody of the child in this or another COLlrt. Plaintiffs have no information of a custody proceeding concerning the child pending in a court of this Commonwealth. Plaintiffs do not know of a person not a party to the procee'dings who has physical custody of the child or claims to have custody or visitation rights with respect to the child. 7. The best interest and permanent welfare of the child will be served by granting the relief requested because Plaintiffs need assurance that paternal grandA'lElt"'W will have the authority to meet the child's health, welfare, and educational needs. Epperso ,hereinafter referred to as Father and Carol A. and Albert Epperson, hereinafter referred to as Paternal Grandparents. WHEREAS, the above-named Mother and Father had born to them the fOllowing child on the fOllowing date: NAME Alexis V. Epperson BIRTH DATE August 6, 1998 AND WHEREAS, the above-named Mother and Father desire to enter into a Stipulation as to the custody of the above-said child and to the partial custody rights of the non-custodial parents while Father is away on military service: NOW, THEREFORE, in order to effectuate the above purpose, the above-named Mother, Father and Paternal Grandparents hereby stipulate that: 1. Mother, Father and Paternal Grandparents shall share legal custody of the child. 2. Paternal Grand shall have primary physical custody of the child. 3. Mother and Father shall have liberal periods of partial custody as agreed upon by the parties. Verney, Esq. , the conciliator, at 4th Floor, Cumberland County Courthouse, Carlisle on Thursday, October 13, 2005 at 8:30/1, rY\, for a Pre-Hearing Custody Conference. At such conference, an effort will be made to resolve the issues in dispute; or if this cannot be accomplished, to define and narrow the issues to be heard by the court, and to enter into a temporary order. All children age five or older mav also be present at the conference. Failure to appear at the conference mav provide grounds for entry of a temporary or permanent order.
