Mislitsky, in the matter of a name change for James M, Jeffries and Barbara Christie Jeffries as follows: I. Petitioner James M, Jeffries ("Petitioner") is an adult individual residing at 226 Pine Road, Mount Holly Springs, P A 17065, 2. Petitioner Barbara Christie Jeffries ("spouse") is an adult individual residing at 226 Pine Road, Mount Holly Springs, PA 17065. 3. Petitioners have resided at the above address continuously for a period in excess of 5 years. 4, Spouse Christie Jeffries joins in the Petition pursuant to 54 Pa, C.S.A. ~ 701 (a.i)(i). 5. James M, Jeffries is the son of Amy Ruth Dietrich ("mother"), formerly Amy Ruth Byers, among other married names. 6, Mother was married to Petitioner's father, James George Katora, who died in or about April 1971, approximately 3 months prior to Petitioner's birth. 7. Mother married Douglas Jeffries in or about 1983 when Petitioner was in the fourth grade of the Newville Elementary School. 8. Petitioners believe and therefore aver that James Jeffries' adoption records have been sealed, or in the alternative, Petitioner has been denied access to such records and advised that information such as adoption records, birth records and the like could not be provided without an order of Court. 9. As a result ofthe averments set forth in paragraph 8 above, Petitioner is unable to provide more specific information, 10. Pursuant to the aforementioned adoption, Petitioner assumed the surname of his adoptive father, "Jeffries," I L Petitioner's adoptive father abandoned the Petitioner and divorced Petitioner's mother in or around 1989. He is now remarried and believed to be residing in north central Pennsylvania. 12, Petitioner has retained his adoptive name despite the aforesaid facts. 13. Petitioner now joins with his spouse and petitions this Honorable Court for a change of name to his birth name, James Michael Katora. 14. Pursuant to 54 Pa.C.S.A. S 701 et sea, Petitioner requests an order scheduling a hearing and compelling publication pursuant to S 701(a.i)(3), 15. In accordance with the requirements set forth in 54 Pa.
