FINAL DECREE
AND NOW, this 2.:1 day of September, 2005, this matter having been called
for a hearing, the Court finds, upon ~~~~vinCing evidence, that Catherine Hoover / y/,/
is an incapacitated person~he suffers from a mental impairment that prevents her c:?I '-
from receiving and evaluating information effectively and prevents her from formulating
and communicating decisions to such a significant extent that she is unable to manage her
financial affairs or meet essential requirements for her physical health and safety on a
daily basis.
The Court appoints Mary Means as Permanent Plenary Guardian of the Person of
Catherine Hoover and M&T Bank as Permanent Plenary Guardian of the Estate of
Catherine Hoover. In the event that Mary Means decides to move Catherine Hoover
from any nursing home facility, she is directed to notify the Area Agency on Aging, in
and for Cumberland County, prior to doing so. y
