TO THE DEFENDANT: The above named plaintiff(s) asks judgment against you for $ 2.301.40 together with costs
upon the following claim (Civil fines must include citation of the statute or ordinance violated):
Expenses incurred by Plaintiff to replace pole located on SR 74 and York Road, South Middleton Township,
Cumberland County, Pennsylvania, consisting of Labor ($1,596.25), Material ($354.86), Employee Expenses
($66.79), and Transportation ($283.50) damaged when negligently struck by a vehicle owned and operated by John
A. Jakomas, Defendant, on October 13, 2002. Although due demand was made by Metropolitan Edison Company,
Defendant refuses to pay.
