Petitioner N a Dong is a sixteen (16) years old minor residing at 100 Stonehedge Drive, Carlisle, Cumberland County, Pennsylvania. 2. Petitioner Bi Chen is an adult individual residing at 100 Stonehedge Drive, Carlisle Cumberland County, Pennsylvania, and is the mother and legal guardian ofNa Dong. 3. Petitioner Na Dong's father, Xing Yi Dong, is deceased. 4. Petitioner Na Dong was born in China, immigrated to the United States in 1992 with her mother, Petitioner Bi Chen, and became a United States citizen in 2002. A true and correct copy of Petitioner Na Dong's Certificate of Citizenship is attached hereto as Exhibit "A" and incorporated herein by reference. 51953.1 5. Petitioner Na Dong is commonly called and known as "Na Na" by her family, friends and teachers. 6. In accordance with 54 Pa.C.S.A. S702, Petitioners Na Dong and Bi Chen desire to legally change Petitioner Na Dong's name to "Na Na Dong." WHEREFORE, Petitioners respectfully request that the Court: a. fix a hearing on this Petition; b. direct that notice be given ofthis Petition and hearing; and c. enter an Order changing Na Dong's name to Na Na Dong.
