PETITION TO CHANGE PERMANENT PLENARY GUARDIAN
OF THE ESTATE
AND NOW COMES THE PETITIONER, the Area Agency on Aging, in and for
Cumberland County, Pennsylvania, by its solicitor, Anthony L. DeLuca, Esquire, who
represents and avers as follows:
1.
The Petitioner is the Area Agency on Aging, in and for Cumberland County,
Pennsylvania, with its office located at 16 West High Street, Carlisle, Pennsylvania.

The incapacitated person is Mary Dugan, age 80, who, until April 8, 2005, had
resided at 308 Mt. Allen Drive, Mechanicsburg, Cumberland County, Pennsylvania and
currently is residing at The Bridges at Bent Creek, Mechanicsburg, Pennsylvania, an
assisted living facility.

On or about April 22, 2005, a Petition for the Appointment of Emergency Plenary
Guardian of the Person and Estate in accordance with 20 P.S.g5513 and for Pennanent

Guardian of the Person and Estate pursuant to 20 P.S. g5511 was filed.

A hearing for the appointment of Emergency Plenary Guardians of the Person and
Estate was held on April 26, 2005 and, at the conclusion of the hearing, Petitioner was
appointed Emergency Plenary Guardian of the Person and Estate of Mary Dugan,
pending a final hearing on this Petition.

The hearing for the appointment of Permanent Plenary Guardian of the Person
and Estate of Mary Dugan was held on May 19, 2005 and an Order of Court was entered
on June 1,2005 appointing the Area Agency on Aging, in and for Cumberland County,
Pennsylvania as Permanent Plenary Guardian of the Person of Mary Dugan and
Pennsylvania Guardianship Association as Permanent Plenary Guardian of the Estate of
Mary Dugan.

Differences have arisen between certain family member(s) and Pennsylvania
Guardianship Association which make the continued guardianship of the estate by
Pennsylvania Guardianship Association difficult.
7.
As a result of said differences, Pennsylvania Guardianship Association has
consented to withdraw as Permanent Plenary Guardian of the Estate.
8.
Keystone Guardianship Services, with an office located at 2941 North Front
Street, Harrisburg, Pennsylvania, has consented to being appointed as Permanent Plenary
Guardian of the Estate of Mary Dugan.
WHERREFORE, the Petitioner respectfully requests that the Court revoke the
appointment of Pennsylvania Guardianship Association as Permanent Plenary Guardian
of the Estate of Mary Dugan and appoint Keystone Guardianship Services as Permanent
Plenary Guardian of the Estate of Mary Dugan.
