Seyfried, by her counsel Friedman & King, P .C., Richard S. Friedman, Esquire, requests that this Court order a change of name for her natural son, Zachary T. Johnson, a minor, pursuant to 54 Pa.C.S.A. 701 et seq., and in support thereof represents as follows: 1. Petitioner is a resident of Cumberland County Pennsylvania, and has resided at the following locations during the five years preceding the filing of this petition: 406 Independence Court Mechanicsburg, P A 17050 (From June 2004 to present) 714 Randolf Street Hanover, P A (From January 2003 to June 2004) 5 McIntosh Drive Thurmont, MD (From 1999 to June 2003) 2. Petitioner is the natural mother of Zachary T. Johnson, a minor. Zachary T. Johnson was born on August 7, 1994, in Pittsboro, North Carolina, as evidenced by a copy of his birth certificate attached as Exhibit "A". 3. Danny Ray Edwards is the natural father of Zachary T. Johnson, a minor, as shown on the birth certificate, attached as Exhibit "A". 4. The natural father has not seen nor contacted Zachary T. Johnson for more than eight (8) years preceding this petition. 5. Petitioner married Carl R. Seyfried on May 31,2003, as evidenced by a copy of the marriage certificate, attached as Exhibit "B". 6. Carl R. Seyfried and Zachary T. Johnson have formed an emotional bond over the years of Petitioner's marriage and, Carl R. Seyfried, as stepfather, has acted in all steads and instances as father to Zachary T. Johnson, since that time. 7. Zachary T. Johnson would like to be known by and take the surname of his stepfather. 8. Petitioner and her husband would like Zachary T. Johnson to be known as Zachary T. Seyfried, to promote family unity and both feel it is in the best interest of the minor. 9. The natural father of Zachary T. Johnson will consent to the change of name. WHEREFORE, petitioner respectfully requests that this Court enter an Order changing the name of the minor from Zachary T. Johnson to Zachary T. Se G VERIFICATION I, Shanda L.
