PETITION TO CHANGE NAME
1. Petitioner is Jennifer Young, who resides at PO Box 408, Mifflin, Mifflin County,
Pennsylvania 17058. She files this Petition on behalf of her minor son, Draven Blaine
Arnett.
2. The minor child has resided at the following address for the five years previous to
filing of this petition: 424 Seventh Street, New Cumberland, Cumberland County,
Pennsylvania 17070.
Although Petitioner currently resides in Mifflin County, her minor son currently lives with
his grandparents in Cumberland County. His grandmother has legal custody of Draven.
5. Petitioner desires and intends to change her son's name to DRA VEN BLAINE
GROSS.
6. Petitioner provides the following reasons for this request:
The parental rights of the minor child's father were terminated in 2004. It is therefore in
the child's best interest that his surname be changed to his mother's maiden name in order
to avoid confusion in his life regarding the origin of his surname inasmuch as Mr. Arnett no
longer has any relationship, legal or otherwise, with the minor child, nor will he in the
future.
7. Petitioner states that no credit has been extended to her or to her son in the last ten
years under the name of Draven Blaine Arnett, and that this change of name is not
requested for the purpose of concealment of identity or for any fraudulent purpose.
WHEREFORE, Petitioner respectfully requests that this Honorable Court enter a
decree changing her minor son's name from DRAVEN BLAINE ARNETT to DRAVEN
BLAINE GROSS.

AND NOW, this 7th day of November, 2005, upon consideration of the attached
Petition To Change Name, a hearing is scheduled for Thursday, January 5, 2006, at 11 :15
a.m., in Courtroom No. I, Cumberland County Courthouse, Carlisle, Pennsylvania.
IT IS FURTHER ORDERED that notice of the filing of said Petition and the
aforesaid date, time and place of the hearing be published once in the Cumberland Law
Journal and a newspaper of general circulation in the County at least 10 days prior to the
hearing; in addition, Petitioner shall comply with all other requirements of C.C.R.P. 5,
and the Act of December 16, 1982, P.L. 1309, ~6 (see 54 Pa. C.S. ~701 [Historical and
Statutory Notes] [Main VolumeD, and 54 Pa. C.S. ~S701 et seq.
