HOWARD, on behalf of her minor child, DIANNE OLIVIA WHITE, and by her attorney, MAX J. SMITH, JR., Esquire, and represents the following: L Petitioner CHERYL L. HOWARD is an adult individual who resides at 640-13 Geneva Drive, Mechanicsburg, Cumberland County, Pennsylvania. 2. Petitioner is the natural mother of DIANNE OLIVIA WHITE, a minor child, born November 15, 1994, who resides with Petitioner at 2912 Butler Street, Harrisburg, Dauphin County, Pennsylvania. Said child has resided with her mother since birth. 3. The father of the Petitioner referenced above is MARK L. WENGER, who resides at 5 Keystone Drive, Mechanicsburg, Cumberland County, Pennsylvania. 4. The said natural parents of the aforesaid minor child have never been married to each other. 5. Petitioner Cheryl L. Howard was married to Dana J. Howard on October 3, 2005, at which time her last name changed from her maiden name, "White" , to "Howard" . 6. Petitioner desires to have Dianne's surname legally changed from White to Howard, so that all family members shall have the same last name. 7. Said MARK L. WENGER has indicated by way of written consent his approval of the child's name change from White to Howard. (See Consent to Name Change attached hereto, marked Exhibit" A" , incorporated herein by reference thereto.) 8. Dianne's last name heretofore has always been" White" , and she has never been known by her father's last name. 9. There are no judgments or decrees of like character of record against the minor child, nor has she ever been convicted of a crime. 10. Dianne, by and through her mother, Petitioner herein, is not requesting to legally change her name to avoid payment of any debts. WHEREFORE, Petitioner prays this Honorable Court to enter a decree changing Dianne's name to DIANNE OLIVIA HOWARD, pursuant to Act of April 18, 1923, P.L. 75, 54 P.S. 1, et~. Said Petition to Change Name contains an inadvertent typographical error, specifically, the address where the minor child resides as being 2812 Butler Street, Harrisburg, Dauphin County, Pennsylvania. Such address should be changed to be 640-13 Geneva Drive, Mechanicsburg, Cumberland County, Pennsylvania. 3. No prejudice to anyone involved in these proceedings inures as a result of the aforesaid suggested corrections to the Petition to Change Name.
