Barrick, an adult individual, owning property located at 347A, 347B and 347C Old Stonehouse Road South, Boiling Springs, Monroe Township, Cumberland County, Pennsylvania, 17007, whose last known mailing address is 347B Old Stonehouse Road South, Boiling Springs, Pennsylvania 17007. James R. Barrick, is the owner of all those certain lots of ground, including any improvements thereon, being more fully described as follows: ALL THAT CERTAIN lot of ground situate in the Village of Churchtown (Allen, P.O.) Monroe Township, Cumberland County, Pennsylvania, more particularly bounded and described as follows: ON the West by Main Street; thence by said street, 35 =? feet, more or less, to High Street; thence by said Street, 200 feet to an alley; thence by said alley, 35 112 feet to Lot No. 7; thence by said Lot No. 7, 200 feet to the Place of BEGINNING. BEING Lot No. 8 of Additional Plan of town lots. HAVING thereon erected a 2 112 story brick dwelling house. BEING the same premises which James E. Barrick, a/k/a James R. Barrick, single individual, and Keith A. Barrick and Susan M. Barrick, husband and wife, by deed dated September 3, 1999 and recorded September 13, 1999 in the Cumberland County Recorder of Deeds Office in Deed Book 207, Page 614 granted and conveyed unto James R. Barrick, single individual, owner. Tax Parcel No. 22-28-2401-107. Monroe Township Municipal Authority hereby files this Municipal Lien for Services and Sewer Rates levied and assessed from July 1, 2003 through and including October 15, 2005. Said Authority services and sewer rates were levied and assessed pursuant to Authority Resolution No. 02-04, same being known as a Resolution of the Board of Monroe Township Municipality Authority imposing user charges to be collected from the owner of each improved property served or to be served by the Authority's sewer system, in conjunction with all prior relevant Resolutions and Ordinances, including, but not necessarily limited to, Monroe Township Ordinance No. 3 of 1978 and Monroe Township Ordinance No. 99-6, as amended; and providing for payment and collection of such user charges. Attached hereto and incorporated herein are -2- the statements forwarded by Monroe Township Municipal Authority to James R. Barrick, for sewerage due to be treated from July 1, 2003 through October 15, 2005, which services total is in the amount of $6,100.74, which sum was duly assessed against. James R. Barrick, and the real property described herein from the date of said billing statements as attached hereto, and for which sum, with interest in the amount of ten (10%) percent., along with penalties and all costs, including attorneys' fees, a municipal lien is claimed against the above-described real property and premises in accordance with the Act of May 16, 1923, P.L. 207, §7, as amended, 53 P.S. §7101, et seq, as amended (Municipal Claims and Tax Liens).
