AFFIDAVIT OF NOTICE
Neoma P. Minium, being duly sworn according to law, deposes and says that she
gave notice of the hearing scheduled on Monday, October 16, 2000 at 2:30 P.M. in Court
Room #3 of the Cumberland County Court House in Carlisle, Pennsylvania, before
the Honorable George E. Hoffer, in the above captioned matter by sending by Certified
Mail, Return Receipt Requested, a copy of the Preliminary Order of Court to all next-of-kin
named in the Petition of the above-captioned person who are sui juris and within the
Commonwealth of Pennsylvania, at the addresses supplied by the institution of which the
alleged incapacitated person was a resident.. A copy has also been served on Joseph A.
Crouse, now residing at a Family Care Home, c/o Roberta Ford, 61 5th Avenue,
Fayetteville, Pennsylvania, 17222.
