Petition filed in this matter, it is ORDERED and DECREED that Amy L. Heller, as Plenary
Guardian of the Estate of Daniel J. Whare, is authorized, pursuant to 20 Pa CSA ~5536(b)(8) to
transfer ownership of Monumental Life Insurance Policies, Nos. MM1895679 and W63056425,
to Amy L. Heller and to designate beneficiaries of said policies as his children, Michael J. Whare
and Amy L. Heller. Furthermore, Petitioner is authorized to use the principal of Daniel J. Whare
to pay for care and medical expenses, burial expenses, and attorney fees.
