On January 16, 2001 Judgement was entered for Blazer Consumer Discount Company and
against Bonnie K. Moore. There was no appeal fried in the Lebanon County Prothonotary's
Office and there is no appeal pending. The judgement states the defendant has 30 days to appeal
the judgement. District Justice Lee R. Lehman certified the judgement on March 22, 2001
stating the 30 day right to appeal had expired and Blazer Consumer Discount Company had won.

Finance verify all the information in this Affidavit of No Appeal to be true & correct.
