Smith, Guardian, respectfully represents: 1. Your Petitioner was appointed Guardian of the Estate of William Bunty by this Court on November 17. 1978, at Number 503 by successorship-ill-office to Bonita 1. Stahl. 2. At that time, the incapacitated person was a resident at Selinsgrove Center. 3. The incapacitated person's domicile at the time of his admission to Selinsgrove Center was 119 S. Hanover Street Carlisle. Pa. 4. The incapacitated person is 50 years of age, having been born on January L 1950. 5. The incapacitated person's military status is: never served in the armed forces. 6. The in7apacitated person's marital status is: never married. 7. Those persons, if any, who are the incapacitated person's next-of-kin, their address and relationship to same, of whom your Petitioner has knowledge are as follows: Mrs. Kathleen Foreman 1050 HofTRoad Hanover, Pa. To the best of Petitioner's knowledge, no institution or individual is willing to be appointed Successor Guardian of the Estate of William Bunty. 11. The incapacitated person has income consisting of: (a) Money due from Social Security Administration in the amount of $ 665.00 per month. 12. The incapacitated person has assets, real and personal, of which your Petitioner is aware in the amount of $ 5.763.01 consisting of: (a) Cash held by the Guardian in the amount of $105.82 as of August 31. 2000..
