PRELIMINARY DECREE

to LENA CACOPARDO, to show cause why she should not be adjudged an in-
competent and a guardian of her estate appointed; hearing to be held in

PETITION FOR APPOINTMENT OF A GUARDIAN
TO THE HONORABLE, THE JUDGES OF THE SAID COURT:
The Petition of Mary T. Lake respectfully represents:
at 106 Hampden Avenue, Camp Hill, Cumberland County, Pennsylvania.

2. That the alleged incompetent is ninety-five (95) years of age and is a i

widow presently residing at Blue Ridge Haven West, 770 Poplar Church Road, I

3. That the next of kin of the alleged incompetent are: two daughters,
Mary T. Lake of 106 Hampden Avenue, Camp Hill, Pennsylvania l70lL and

Anna C. Foster of 214 N. Thirtieth Street, paxtang, Harrisburg, Penn s ylvania.1

and one son, Joseph J. Cacopardo, 3305 Lawndale Avenue, Fort Worth, Texas
76133.
4. That the gross value of the alleged incompetent's estate is approxi-
ately Eight Thousand Dollars---($8, OOO)---and the net income from all sourc s
is approximately Two Thousand Six Hundred Dollars- --($2, 600)-- -per annum.
5. That the alleged incompetent has never been a member of the Armed
, Services of the United States and is not receiving any benefits from the United
States Veteran's Administration.
6. That the alleged incompetent is unable to manage her property and
lacks sufficient capacity to make or communicate responsible decisions con-
cerning her person due to mental deficiencies and the infirmities of old age.
7. That the proposed guardian is your Petitioner, the alleged incom-

determine the competency of the alleged incompetent.

incompetent with notice thereof to her next of kin and to such other persons as
the Court may direct, to show cause why she should not be adjudged an incom-
petent and a guardian of her estate appointed.

I
I am a United States citizen over the age of 21 and am able to read.. writel
and speak English. I am domiciled at 106 Hampden Avenue, Camp Hill,
Cumberland County.. Pennsylvania.
I am not the fiduciary or an officer or employee of any corporate fidu-
ciary of an estate in which the alleged incompetent has an interest.. nor am I
the surety or an officer or employee of the corporate surety of such a fiduciar .
Furthermore.. I have no interest adverse to the alleged incompetent and
do not reside in the same household with her..
I therefore agree to accept the appointment as Guardian of the estate of
the alleged incompetent.
