PETITION FOR APPOINTMENT OF A GUARDIAN
TO THE HONORABLE, THE JUDGES OF THE SAID COURT:
The Petition of Mary T. Lake respectfully represents:
1. That Petitioner is the daughter of the alleged incompetent and resides
at 106 Hampden Avenue, Camp Hill, Cumberland County, Pennsylvania.
2. That the alleged incompetent is ninety-five (95) years of age and is a
widow presently residing at Blue Ridge Haven West, 770 Poplar Church Road,
Camp Hill,. Pennsylvania.
3. That the next of kin of the alleged incompetent are: two daughters,
Mary T. Lake of 106 Hampden Avenue, Camp Hill, Pennsylvania 17011'. and
Anna C. Foster of 214 N. Thirtieth Street, Paxtang, Harrisburg, Pennsylvani
and one son, Joseph J. Cacopardo, 3305 Lawndale Avenue, Fort Worth, Texas
76133.

4. That the gross value of the alleged incompetent's estate is approxi-
ately Eight Thousand Dollars---($8, OOO)---and the net income from all sourc s
is approximately Two Thousand Six Hundred Dollars---{$2, 600)---per annum.
5. That the alleged incompetent has never been a member of the Armed
Services of the United States and is not receiving any benefits from the United
States Veteran's Administration.
. .6. That the alleged incompetent is unable to manage her property and
lacks' sufficient capacity to makeorcommunicate responsible decisions con-
cerning her person due to m ental deficiencies and the infirmitfes of old age.
7. That the proposed guardian is your Petitioner, the alleged incom-

8. That the proposed guardian has no interest adverse to the alleged
incompetent.
9. That no court has ever assumed jurisdiction in any proceeding to
determine the competency of the alleged incompetent.
10. That the alleged incompetent has no guardian already appointed. .
11. Petitioner respectfully prays that a Citation be directed to the allege
incompetent with notice thereof to her next of kin and to such other persons as
the Court may direct. to show cause why she should not be adjudged an incom-
petent and a guardian of her estate appointed.

NOTICE
TO:
Please take notice that upon petition by Mary T. Lake to the said
Court, it has decreed that a hearing on the petition shall be held to inquire
into the alleged incompetency of Lena Cacopardo.
The hearing will be held on November 15, 1978 at 9:45 a. m. in Court-
room No. 1, Cumberland County Courthouse, Carlisle, Pennsylvania. Copies
of the petition and preliminary decree for hearing are attached hereto.
