Enclosed please find a copy of a Petition to Change
Beneficiary on Life Insurance Policies. This matter was continued
from the last time we were in Court. This matter will now come
before the Court on Wednesday, May 6, 1992 at 9:30 A.M. in
Courtroom No. 1 of the Cumberland County Courthouse be'fore the
Honorable Harold E. Sheely.

Nancy Neast, being duly sworn according to law, deposes and
says that she gave notice of the hearing scheduled for
May 6, 1992
Cumberland
at 9:30 A.M. in the Orphans' Court Room of the
County Courthouse in Carlisle
Pennsylvania
on the above-captioned matter by sending by Certified Mail, Return
Receipt, a copy of the Court Citation and a letter of explanation
from Thomas Blazusiak, Assistant Counsel, Northeastern Region,
Department of Public Welfare, to all next-of-kin named in the
Petition of the above-captioned person who are sui juris and
within the Commonwealth of Pennsylvania, at the addresses supplied
by the institution of which the alleged incompetent is a
resident. She sent the same by first-class mail to those outside
the Commonwealth. A copy of the Citation and the letter are
attached: having been sent on April 6, 1992.
