PETITION FOR APPOINTMENT OF GUARDIAN FOR A MINOR' i'~_,) C,,) AND NOW, comes Petitioners, Gloria A. Anderson and David L. Anderson, l>y! i'.,,) and through their counsel of record, Bradley L. Griffie, Esquire and the law firm of- "-''J Griffie & Associates, and petitions the Court as follows: 1. Your Petitioners are Gloria A. Anderson and David L. Anderson, adult individuals currently residing at 810 Greenspring Road, Newville, Cumberland County, Pennsylvania. 2. The minor at issue is Joshua Lee Anderson, of810 Greenspring Road, Newville, Cumberland County, Pennsylvania, age 12, born November 26, 1993. 3. The natural parents of the minor, Joshua Lee Anderson, are your Petitioners, Gloria A. Anderson and David L. Anderson. 4. The minor's parents consent to this Petition in that they are the Petitioners and have requested the appointment of the Guardian, hereinafter set forth. 5. The necessity or reason for appointment of Guardian in this matter has arisen due to the fact that the minor is a named beneficiary on certain life insurance policies with Prudential Financial on the named insured, Victor S. Russell, deceased. 6. The said Victor S. Russell passed away on May 5, 2005 as the insured under three life insurance policies with Prudential Financial, as evidenced by the enclosed correspondence dated May 11, 2005 to your Petitioners. \11 ~ 7. The minor at issue has not had a guardian appointed for him in any other proceedings. 8. The proposed Guardian for the child, is Adams County National Bank/Trust Department of 16 Lincoln Square, Gettysburg, Adams County, Pennsylvania, a Pennsylvania corporation, which has consented and agreed to accept the appointment of Guardian through Paul H. Ketterman, Vice-President and Senior Trust Officer for Adams County National Banle 9. Your Petitioners and the proposed Guardian have no interest adverse to that of the minor at issue. 10. The minor has no assets of any nature beyond those assets to be received upon the disbursement of the death benefit proceeds from the Prudential Financial life insurance policies on the life ofthe late Victor S. Russell as referenced above. 11. From the limited information that Petitioners and their counsel have been able to secure from Prudential Financial, the exact amount of the life insurance death benefits are unknown at this time. 12. The minor at issue is not involved in any litigation which will result in the payment for disbursement of funds. 13. As the parents of the minor are not now, nor have they been, in the military service with the United States Government, there is no obligation to give notice to the United States Veterans Administration or its successor. 14. Adams County National Bank/Trust Department has executed their consent to serve as Guardian in this matter, which consent is attached hereto and incorporated herein by reference as Exhibit "A." WHEREFORE, Petitioners requests your Honorable Court to enter an Order naming Adams County National Bank/Trust Department as Guardian of the estate of Joshua Lee Anderson, born November 26, 1993.
