PETITION FOR CHANGE OF NAME
TO THE HONORABLE, THE JUDGES OF SAID COURT:
The Petition of Kendra S. Wiederhold by her undersigned attorney, respectfully
represents as follows:
1. Kendra S. Wiederhold is the birth mother of Petitioner, and after her recent
marriage adopted the surname of her husband, Wiederhold.
2. Since her birth on June 1, 1997, Petitioner, Carmen Shae Stumpo has carried the
surname of her natural father. For her whole life, Petitioner has been raised by and lived with
Petitioner's mother. Petitioner wishes to make her surname consistent with the surname of
Petitioner's mother and Petitioner's step-father.
3. During the last five years, petitioner has resided at 316 Virginia Rd,
Mechanicsburg, Cumberland County, Pennsylvania, at 824 Meadow Lane, Camp Hill,
Cumberland County, and at 2 Village Court, Mechanicsburg, Cumberland County, Pennsylvania.
4. There are no judgments or decrees of like character of record against the
Petitioner.
WHEREFORE, Petitioner requests the Court, pursuant to the Act of December 16, 1982,
P.L. 1309, No. 295, ~2, 54 Pa. C.S.A. ~702, for the following;
a. To fix a hearing on this Petition;
b. To direct that notice be given of this Petition and hearing; and
c. To enter a decree changing the Petitioner's name from Carmen Shae Stumpo
to Carmen Shae Wiederhold.

AND NOW, this 30th day of January, 2006 on hearing ofthe within Petition and
on motion of Francis X. O'Brien, attorney for the petitioner, and on presentation of proof
of publication of notice as required by law, proof of personal service on the minor's
natural father, Anthony Stumpo, and being convinced that this name change is in the best
interest of Carmen Shae, the unemancipated minor, and it appearing that there is no legal
or other objection to the granting of the prayer of the petition,
IT IS ORDERED AND DECREED that the name of the unemancipated minor be
and is hereby changed to Carmen Shae Wiederhold.
