The names of the minors for whose Estates guardians are being requested are as follows: a. Le Dora E. Allen is 15 years of age and was born on February 3, 1990, and resides at 214 Rosemont Avenue, New Cumberland, Pennsylvania, 17070 (hereafter referred to as "Le Dora"); and b. Katharine J. Allen is 13 years of age and was born on September 8, 1992, and resides at 214 Rosemont Avenue, New Cumberland, Pennsylvania, 17070 (hereinafter referred to as "Katharine"). 2. The minors do have a living parent. The living parent, Judy A. Allen, the minors' adoptive mother, resides at 214 Rosemont Avenue, New Cumberland, Pennsyl- vania, 17070 (hereafter referred to as the minors' "Mother"). The minors' adoptive father died on March 31,2005 without appointing a guardian for the Estate of either minor. 3. This Petition is filed by the following "Petitioners:" a. The minors' Mother, on behalf of Katharine, and is an adult person with whom the minors reside, and b. Le Dora, a minor over the age of 14 years old. 4. Petitioner Le Dora has the consent of her Mother and surviving parent of the minors to the proposed guardians requested in this Petition. 5. The minors have no guardian of their Estates, and the appointment of the proposed guardians for their estates is necessary for the minors are entitled to receive the death benefits from the Prudential Insurance Company of America Annuity Services Group Life Insurance Policy #G-91314 on the life of their deceased father Donald Allen, as the name beneficiaries with a face amount of Thirty-Nine Thousand Dollars ($39,000.00) for each of the minors. 6. The name and address of each proposed guardian, the age of any proposed individual guardian, and the relationship of any proposed individual guardian to the mi- nors, if any, are as follows, and the consent of each proposed guardian is attached: a. Judy A. Allen, 59 years of age and mother of the minors; and b. Michael S. Zeigler, approximately 45 years of age and friend of the minors. 7. Any proposed individual guardian is 18 years of age or older, has no interest that is adverse to the of the minors, and is not a fiduciary or surety, or an employee of a corporate fiduciary or surety, of an estate in which the minors have an interest, and any proposed corporate guardian is authorized to act as a fiduciary in the Commonwealth, has 102575 2 no interest that is adverse to that of the minors, and is not a fiduciary or surety of an estate in which the minor has an interest. 8. The following is an itemization of the assets of the Estates of the minors and their location, approximate value, and income, if any: a. Le Dora E. Allen - Prudential Insurance Company of America Annu- ity Services Group Life Insurance Policy Group Number 91314, Certificate Number 194346678 currently being held by Prudential Financial in an interest-bearing ac- count (approximately 1 % per year) awaiting appointment of a guardian of the mi- nor's Estate. The death benefit value of the annuity is $39,000.00. A true and cor- rect copy of the Claim Settlement Certificate is attached hereto and more fully set forth as Exhibit" A." b. Katharine J. Allen - Prudential Insurance Company of America Annu- ity Services Group Life Insurance Policy Group Number 91314, Certificate Number 194346678 currently being held by Prudential Financial in an interest-bearing ac- count (approximately 1 % per year) awaiting appointment of a guardian of the mi- nor's Estate. The death benefit value of the annuity is $39,000.00. A true and cor- rect copy of the Claim Settlement Certificate is attached hereto and more fully set forth as Exhibit "B." 9. Notice to the United States Department of Veterans' Affairs is not required. 10.
