I. Plaintiff is Dennis J. Kelleher, IV, an adult individual, residing at 132 Partridge
Circle, Carlisle, Cumberland County, Pennsylvania 17013.
2. Defendant is Judith P. Treffinger, an adult individual, residing at 6 Stewart Drive,
Carlisle, Cumberland County, Pennsylvania 17013.
3. The parties are the parents of one minor child: Genna M. Kelleher, born April 25,
1988.
The child was born in wedlock.
The child is presently in the custody of Plaintiff at 132 Partridge Circle, Carlisle,
Pennsylvania.
During the past five years, the child has resided with the following persons at the
following addresses:

The natural father of the child is Dennis J. Kelleher, IV, residing at 132 Partridge
Circle, Carlisle, Cumberland County, Pennsylvania.
The natural mother of the child is Judith P. Treffinger, residing at 6 Stewart Drive,
Carlisle, Cumberland County, Pennsylvania.
She is married to Hemy Treffinger.
4. The relationship of the Plaintiff to the child is that of natural father. The Plaintiff
currently resides with the following persons:

5. The relationship of the Defendant to the child is that of natural mother. The
Defendant currently resides with the following persons:

Henry Treffinger
Relationshio
Husband
6. Plaintiff has not participated as a party or witness, or in any other capacity in other
litigation, concerning the custody of the child in this or in any other Court.
Plaintiff has no information of a custody proceeding concerning the child pending
in a court of this Commonwealth.
Plaintiff does not know of a person not a party to the proceedings who has
physical custody of the child or claims to have custody or visitation rights with respect to the
child.
7. Each parent whose parental rights to the child have not been terminated and the
person who has physical custody of the child have been named as parties to this action. All other
persons, named below, who are known to have or claim a right to custody or visitation of the
child will be given notice of the pendency of this action and the right to intervene.
NONE.
WHEREFORE, Plaintiff requests your Honorable Court to grant him shared legal and
primary physical custody ofthe child.

it is hereby directed that parties and their respective counsel appear before Jacqueline M. Verney, Esq. ,the conciliator,
at 4th Floor,,~~'!I1J,er:land CO'!!'ty Courthouse, C~!lisle_ on u..,...I~.esday, January 17,2006 at 8:30 AM
for a Pre-Hcaring Custody Conference. At sllch conference, an effort will be made to resolve the issues in dispute; or
if this cannot bc accomplished, to define and narrow the issues to be heard by the court, and to enter into a temporary
order. All children age five or older may also be present at the conference. Failure to appear at the conference may
provide grounds for entry of a temporary or permanent order.
The court hereby directs the parties to furnish any and all existing Protection from Abuse orders,
Special Relief orders, and Custody orders to the conciliator 48 hours prior to scheduled hearing.

l. The Father Dennis J. Kelleher, IV and the Mother, Judith P. Treffinger
shall have shared legal cu tody of Genna M. Kelleher, born April 25, 1988. Each parent
shall have an equal right, 0 be exercised jointly with the other parent, to make all major
non-emergency decisions ffecting the Child's general well-being including, but not
limited to, all decisions re arding her health, education and religion.
I. Mother sh I have the following periods of physical custody:
A. Beginn ng February 13, 2006, two overnights every week Monday
throug Thursday, consecutive or non-consecutive, unless Genna
refuses r opposes overnights.
B. Such ot er times as the parties and Genna agree.
2. Father shal have physical custody of the child at all other times.
3. This Order' s entered pursuant to an agreement of the parties at a Custody
Conciliation Conference. he parties may modify the provisions of this Order by mutual
consent. In the absence 0 mutual consent, the terms of this Order shall control.
