Before me, the subscriber, a Notary Public in and for said Commonwealth and
County, personally appeared Richard W. Collins, Executor of the Estate of
Charles H. Collins, Sr., late of the Borough of Mechanicsburg, County of
Cumberland and State of Pennsylvania, deceased, \vho, being duly sworn
according to law, deposes and says that the foregoing Account is true and
correct to the best of his knowledge and belief, and that the hereunto
attached statement contains the names (*) of all unpaid claimants who have
given written notice of their claims to the Accountant or to William L.
Sunday, attorney, as well as the names (**) of all persons known to the
Accountant to have an interest in the Estate as beneficiary, heir or next of
kin, and that they have been given notice of the time when and place where
said Account will be presented for confirmation and distribution as required
by Section 3503 of the Probate, Estates and Fiduciaries Code and the Rules of
Court prescribed thereunder.

STATEMENT OF THE NATURE AND REASONS FOR THE PROPOSED AWARD
Charles H. Collins, Sr. died October 4, 1978 after having made a Last
Will and Testament in writing dated September 14, 1957, wherein and whereby
he appointed his son, Richard W. Collins, Executor, gave all his personal
property unto his wife, Evelyn M. Collins, and gave and devised his real
estate known and numbered as 118 East Locust Street, Mechanicsburg, PA unto
his wife Evelyn M. Collins, for and during her natural life and at her death
he gave and devised the same unto his sons, Charles H. Collins, Jr. and
Richard W. Collins.
He further provided that should his said \.life remove
from the house during her natural life, that his Executor should sell and
convert said real estate into money and distribute the proceeds thereof unto
his wife, Evelyn M. Collins, and his sons, Charles H. Collins, Jr. and
Richard W. Collins, equally, share and share alike.
Evelyn M. Collins, Charles H. Collins, Jr. and Richard W. Collins all

survived the decedent, and Evelyn M. Collins occupied the said real estate
until June of 1991 at which time she elected to remove from and vacate said
real estate and directed Richard W. Collins, Executor, to sell the same and
distribute the proceeds to herself, Richard W. Collins and the heirs-at-law
of Charles H. Collins, Jr. who died October 19, 1984.
The real estate was sold and conveyed on October 11, 1991 and a search
began for the persons entitled to the share of Charles H. Collins, Jr.,
deceased.
Finally, a true and correct copy of the Last Will and Testament of
Charles Harold Collins, Jr., as certified on October 1, 1992 by Robert E.
Lockwood, Clerk of the Court of Broward County, Florida, was received by the
Estate of Charles H. Collins, Sr.
The certification stated " . . . that it is a PURP(Rl'EI) Last Will and
Testament, that it (HAS) (HAS ~) been proven, and HAS lIDT BEEN ADMITTED 'ill
Probate and Record in and for Broward County, but is merely DEPOSITED with
the Circuit Court pursuant to the requirements of Section #732.901, Florida
Statutes."
A Family Settlement Agreement was prepared wherein an Account was stated
and was signed by Richard W. Collins on December 15, 1992 and by Evelyn M.
Collins on or before January 12, 1993.
On January 12, 1993 the Family
Settlement Agreement was mailed to Florida for execution by the Executrix of
the Estate of Charles H. Collins, Jr.
On April 21, 1993 Evelyn M. Collins, one of the distributees died.
No response has been received from the Estate of Charles H. Collins, Jr.
to this date.
Therefore, this Account is filed to authorize the distribution of said
estate in accordance with the terms of said Will.
