AFFIDAVIT OF MAILING NOTICE OF ENTRY OF LIEN
Delores Derick, being duly sworn according to law, deposes and says that she is
the supervisor of the Lien Section of the Office of Chief Counsel, Commonwealth of
Pennsylvania, Department of Labor and Industry, and that as such makes the following
affidavit.
That on June 5, 2006, a Notice of Entry of Lien and Intent to Execute, was mailed
by certified mail, return receipt requested as provided by Section 308.1 of the
Pennsylvania Unemployment Compensation Law, Act of December 5, 1936, P.L. (1937)
2897, as amended, addressed to Let's Move It Inc., Suite 101, 1071 Harrisburg Pike,
Carlisle, PA, 17013 the last known post office address of the employer.
This affidavit is made subject to the penalties of 18 Pa. C.S. § 4904 relating to
unworn falsification to authorities.
