find a Petition for Adjudication of Incompetency for Appointment
of Guardian of the Estate and Person in Accordance with 20 Pa.
Cons. Stat. ~5511 and a Preliminary Decree and a Final Decree in
the above-stated matter. Also enclosed are two checks: one in the
amount of $32.00 for the filing fee in the Register of wills
Office; and one for the Sheriff's Department for service.
Would you please time-stamp the enclosed copy of this letter and
return that copy to us in the enclosed stamped self-addressed
