He is a licensed or licensable, practicing psychologist, who is employed at the State Center or Mental Retardation Unit known as Laurelton Center located at Laurelton, Pennsylvania 17835 where on 10/14 , 19~, he examined a resident therein, viz: (Name) Frank Edgett (Age) 20 ( S ex ) M .who was admitted from 1002 N. College Street, carlisle, Penna. on September 18 , 1911-, with a history of moderate mental retardation At the examination of the following symptoms were observed Mr. Edgett could identify coins, but he could not add or make change from which the diagnosis made is: Mr. Edqett is not competent to manage his own funds ~ ~ , with a prognosis as follows: Mr. Edgett will not likely acquire requisite skills to be financially competent in the near future 2.
