INVENTORY AND APPRAISEMENT

incompetent, being duly sworn according to law, deposes and says
that:
1. The items appearing in the following Inventory and
Appraisement include all of the personal assets of said incompetent,
which have come into their possession.
That the valuation placed opposite each item of said Inventory
represents its fair value and/or carrying value as of the date of
the Order of Court dated December 20, 1978, appointing Dauphin
