Addendum - Verified Petition For Name Change
9. Petitioner's grandfather, now deceased, served in the United States Navy during
World War II. Petitioner was commissioned as a second lieutenant in the United S1ates
Marine Corps in August 2005. Upon successful completion ofthe bar examination,
Petitioner will be ordered to active duty. Petitioner will be his grandfather's only
surviving offspring (child or grandchild) to ever serve in the military. Petitioner desires
to honor his grandfather and continue his grandfather's legacy of service to our country
by assuming his grandfather's surname.
