On September 7,2005 Judge Kevin A. Hess appointed the Cumberland County
Office of Aging Emergency Guardian of Person for Ray W. Wingard. Following this
appointment, Mr. Wingard was admitted to Claremont Nursing and Rehabilitation
Center because neither he nor his spouse were able to adequately care for him.
On September 30, 2005 the Cumberland County Office of Aging was appointed
permanent Guardian of Person.
At the time of his admission, Ray Wingard was diagnosed with Alzheimer's Disease,
anxiety and aggressive behaviors. During his time at Claremont, client refused to
participate in activities and several times exhibited aggressive behavior toward staff.
Mr. Wingard was visited several times by Office of Aging Staff and was taken
Christmas presents on December 22, 2005.
On December 23, 2005, I, as the On-Call worker for the Office of Aging, was advised
that Mr. Wingard was not doing well. He had been unable to maintain an adequate
oxygen level in his blood. I notified the family and recommended they visit, which
they did. Mr. Wingard died on December 25,2005. The death certificate lists
microvascular dementia and Alzheimer's as the causes of death.

Report from Guardian of Estate
Cumberland County Office of Aging

On September 7, 2005 Judge Kevin A. Hess appointed the Cumberland County
Office of Aging Emergency Guardian of Estate for Ray W. Wingard. On September
30, 2005 the Cumberland County Office of Aging was appointed permanent
Guardian of Estate. Client was transferred from Holy Spirit Hospital to Claremont
Nursing and Rehabilitation Center.
Following the appointment, the Cumberland County Office of Aging opened a
guardianship account for Mr. Wingard at M& T Bank. The Office also applied to the
County Board of Assistance on his behalf and Medical Assistance became effective
September 6, 2005. The Board of Assistance directed that out of his monthly
pension check, $1448.63 be given to Mrs. Wingard who was still living in the
community and $570.48 be given to the nursing home. Because Mrs. Wingard
received and spent the September check, the nursing home portion was deducted
from her portion of the October check.
The settlement for the sale of the Wingard's townhouse was on December 12, 2005.
His half of the proceeds was $8,873.03. Funeral arrangements were made from
these funds with $5000.00 going to the Sullivan Funeral Home on December 23,
2005. Payments made to Credit Plus, a collection agency, were for a medical bill
incurred before our involvement.
Mr. Wingard died on December 25, 2005. Attached is a spreadsheet showing the
activity in Mr. Wingard's guardianship checking account. The balcmcein the account
at the time of his death was $4008.30.
