Cons. Stat. 9701 et seq. and in support thereof makes the following averments: I. Petitioner is an adult individual residing at 14 Cherokee Drive, Shippensburg, Cumberland County, Pennsylvania. 2. Petitioner was born James Lowell Armstrong on August 19, 1960, in Santa Clara County, California. 3. Petitioner has been known as Jamie Lowell Armstrong all his life, and important documents, including his driver's license and Discharge from Active Duty, reflect his name as Jamie Lowell Armstrong. 4. Petitioner has applied for his HAZMA T license and passport. He is unable to obtain the license and passport because his license and other important papers do not match his birth certificate. s. Petitioner desires to change his name to Jamie Lowell Almstrong so as to obtain his license and passport, as well as finalize the process of formally identifYing himself by his preferred name. 6. Petitioner has resided at 14 Cherokee Drive, Shippensburg, Cumberland County, Pennsylvania, since Mayof2001. Prior to that, Petitioner resided at 259 West Ridge Street, Carlisle, Cumberland County, Pennsylvania, from May of 1998 to May of 200 I. 7. There are no judgments or decrees of like character of record against the Petitioner. WHEREFORE, Petitioner respectfully requests that this Honorable Court: a. fix a hearing on this Petition; b. direct that notice be given of this Petition and hearing; and c. enter a decree changing Petitioner's name from JAMES LOWELL ARMSTRONG to JAMIE LOWELL ARMSTRONG.
