Philip R. Murray ("Disclaimant") respectfully states that:
1. Marian N. Murray ("Decedent") died testate on July 14, 2005, a resident of Camp
Hill, Cumberland County, Pennsylvania, and Letters Testamentary were granted on her estate to
Philip R. Murray by the Register of Wills of Cumberland County on September 19, 2005.
2. Under Paragraph Fourth of her Last Will and Testament, Decedent leaves all of her
personal property to her husband, Philip R. Murray.
3. Said personal property includes a brokerage account administered by TD Waterhouse
bearing the account number~ACC+ t!:C)ol - 'D~(o2.. .
4. Disclaimant desires to disclaim his interest in said account and in accordance with
the terms of Paragraph Fourth ofthe Decedent's Last Will and Testament, said disclaimed property
shall pass to the Unified Credit Trust as established under said Last Will and Testament.
NOW, THEREFORE, intending to be legally bound, the undersigned, Philip R. Murray,
does hereby disclaim the interest as outlined above in the Estate of Marian M. Murray to which he
is entitled under the Last Will and Testament of Marian M. Murray.
