Petitioner Teresa Gordon is an adult individual residing at 1612 Cressman Circle, Mechanicsburg, PA 17055. 2. Petitioner, Teresa Gordon, is parent and legal guardian of Minor Petitioner, Kaitlyn Gordon whose date of birth is September 20, 1988. 3. Respondent, Giant Food Stores, LLC operates a retail grocery store at 255 Cumberland Parkway, Mechanicsburg, P A 17055. 4. On or about October 2,2001, Minor Petitioner, Kaitlyn Gordon, resided with Teresa Gordon. 5. On or about October 2,2001, Minor Petitioner, Kaitlyn Gordon, was at Giant Food Stores Pharmacy Department with her mother, Teresa Gordon. 6. Minor Petitioner, Kaitlyn Gordon, was to pick up a prescription for pain medication due to Minor Petitioner having 3 wisdom teeth pulled but instead received penicillen. 7. Minor Petitioner, Kaitlyn Gordon, did not realize the mix up until about 5 days later. During these five days, KaitIyn suffered increased pain from not taking the proper medication. 8. The parties have agreed to settle minor Plaintiff's claim for $900.00. 9. Since petitioner is not represented by counsel this Petition has been prepared by counsel for the Respondent, who was not a participant in the negotiation of the settlement. WHEREFORE, Petitioner respectfully request this court to approve the foregoing Compromise on behalf of Minor Petitioner, Kaitlyn Gordon.
