TO THE HONORABLE, THE JUDGES OF SAID COURT: The Petition of JAKE N. BRANDT respectfully represents, as follows: 1. Petitioner JAKE N. BRANDT is a minor individual who was born on December 17, 1991, and who resides at 115 Shirley Lane, Boiling Springs, South Middleton Township, Cumberland County, Pennsylvania 17007. 2. The parents of Petitioner, GARY E. BRANDT and MARY ANNA SENEK BRANDT, resided together with Petitioner at the aforesaid address until the death of the mother of Petitioner on November 10,2005. 3. The mother of P~titioner died intestate, leaving Petitioner as beneficiary of WAYNE F. SHADE Attorney at Law 53 West Pomfret Street Carlisle, Pennsylvania 17013 value of $40,000, an interest in the residue of the estate of her father, Walter T. Senek, with an approximate value of $100,000 and U. S. savings bonds with an approximate total value of $2,000. 4. Petitioner was not named in the Last Will and Testament of Walter T. Senek, but, rather, is entitled to a share of that Estate as an intestate heir of his mother who was the sole residuary heir of her father's Estate. 5. The mother of Petitioner did not designate a guardian of the aforesaid assets for Petitioner. 6. The proposed Guardian of the estate of Petitioner is ORRSTOWN BANK, with offices at 427 Stonehedge Drive, Carlisle, Pennsylvania 17013. 7. The acceptance of the proposed Guardian is attached hereto as Exhibit "A" and incorporated herein by reference as though fully set forth. 8. The proposed Guardian has no interest adverse to the minor and is not the fiduciary or an officer or employee of the corporate fiduciary of an estate in which the -2- WAYNE F. The consent of Gary E. Brandt is attached hereto as Exhibit "B" and incorporated herein by reference as though fully set forth.
