Parents: The father of the Petitioner is named James Robert Mitchell (bom February 1.1920), and he resides at 382 Locusffleld Road, East Falmouth, MA. The mother of the Petitioner is named Gertrude Elsie Boyer Mitchell (born October 3, 1916) and resides at Royal Megansett Nursing home. 5. Reason for Change of Name: Personal preference. 6. Legal & Creditor Status: Petitioner does have outstanding judgments and is involved in a pending legal action, but has !!m! been convicted of a crime. 7. Additional Declarations: In addition to the foregoing statements, the petitioner further declares: In October of 2000, Joseph Anthony Larkin misappropriated funds from the Petitioner in an amount exceeding $400,000.00 through misused Power of Attorney for personal gain and fraudulently charged over $35,000.00 on Petitioners credit cards. Petitioner is working with Geoffrev Birinaer of MidPenn Legal, Carlisle.
