Township of Hampden ("Claimant") files this Municipal Claim against Roy Singer and
atricia A. Singer ("Owners") and all that certain tract of land including improvements thereon,
wned by the Owners and described as follows:

or unpaid tapping fees which were charged effective January 26,2006 pursuant to Hampdcn
ownship Code of Ordinances and a tapping fee assessment agreement of the Owners attached
ereto and incorporated herein as "Exhibit A".
Exhibit A attached hereto is a statement of the kind and character of the services so
endered and the prices charged therefor, said statement being a true and correct copy of the
wners' account with the Claimant, which sum of $5,150.00 was duly asscssed against the above
wners and premises beginning January 26,2006 and for which sum, including interest, costs
and attomey's fees, a lien is claimed against the above premises in accordance with the
Municipal Authorities Act of June 19, 2001, P.L. 287, 53 P. S. S 5601, et seq., as supplemented
and amended and the Municipal Claims and Tax Lien Act of May 16, 1923, P.L. 207, 53 P.S S

PINE RUN TAP FEE PAYMENT PLAN AGREEMENT

TOTAL TAP FEE $5,150.00
MUNICIPAL LIEN $ 119,00
Request is hereby made to pay tile above referenced Pine Run Tap fee of$5,150.00 in 21
quarterly installments over a five (5) year period based on the terms that follow, The
unpaid balance will bear interest of eight percent (8%) per annum. Said payment shall be
included in the Township qua.rterly Utility Bill and will be due ,within 30 days of receipt.

The payment pla.n must be paid in full prior to the property owner/resident applying for a
bUilding pennit for any other property improvements. By participating in the payment
plan, all owners agree that their property will be subject to a mUllicipal li.en in an affiolUlt
equal to the total tapping fee, interest and cost, including the Township's attorney fees,
Upon failure to pay any installment and interest within 30 days after it shall become due,
the entire unpaid balance of the tap fce shall become due and payable, together with
accrued interest. All legal costs incurred by the Township in filing the municipal lien and
collecting any amounts due shall be paid by the Owners. The balance may be paid in full
at any time.
This request, if made, must be signed by all owners of the property and must be filed with
in 30 days after receipt of the Hampden Township Sewer Connection Notice.
By signing below, the Owner(s) agree to the terms set forth above.
