That your Petitioner's second wife was one Diane Geist Wertz, who resides at 206 Walnut Bottom Road, Carlisle, PA 17013. 7. That your Petitioner's children, Thea Meli and Rebekah Meli, reside at 727 East Danaher Street, Ludington, MI 49431. II 8. That your Petitioner desires to change his name for more appropriate indication of gender upon gender reassignment by retaining his surname and substituting a female first name and middle name for his current male first name and middle name (Exhibit B). 9. That your Petitioner has no fraudulent purpose in changing his first and middle names, having no creditors (other than current credit cards, all outstanding balances on which he intends to honor) nor judgments, decrees or other process outstanding against him.
