Attached hereto is a statement of the kind and character of the services so rendered and prices charged therefor, said statement being a true and correct copy of the Owner's account the Claimant, which sum of $578.71 was duly assessed against the above Owner and ises beginning January 1, 2005 and for which sum, which includes interest, costs and a lien is claimed against the above premises in accordance with the Municipal Act of June 19, 2001, P.L. 287, 53 P.
