" On May 25,2005 Guardianship of Person for Mr. Moffitt Moffitt was granted by Judge Edward E. Guido to the Cumberland County Office of Aging. Prior to this appointment, on April 22, 2005 Judge Guido ordered that Mr. Moffitt be taken into the custody of the Office of Aging in order for him to have appropriate physical and mental health evaluations. This was done under the Older Adults Protective Services Act. Following the hearing, Mr. Moffitt was taken to the Emergency Room at Holy Spirit Hospital for medical tests. He became combative to the point he was admitted to the Holy Spirit Behavioral Health Unit under a 302 commitment. During this admission, mental illness was ruled out but Mr. Moffitt was diagnosed with hepatitis C, liver cirrhosis and dementia. On June 8, 2005 Mr. Moffitt was admitted to the West Shore Nursing and Rehabilitation Center's dementia unit. This was the only facility that was willing to take Mr. Moffitt. As part of the admission process, the Office of Aging applied for Medical Assistance. This was granted. At time of admission, Mr. Moffitt was able to walk around the unit and communicate with staff. Initially, he was able to feed himself, although he had difficulty. He especially liked to listen to music and play checkers. Mr. Moffitt was later diagnosed with an enlarged spleen. By mid November Mr. Moffitt began to decline quite rapidly and on December 6, 2005 he was moved off the dementia unite because he could no longer feed himself and was incontinent of bladder. He was also very lethargic. On January 3, 2006 Mr. Moffitt was admitted to Holy Spirit Hospital. We received a call from the attending physician the next day. He told us Mr. Moffitt was very ill with end stage liver disease, a urinary tract infection, dehydration and was unresponsive with possible liver failure. The doctor told us his chance of recovery was a "long shot" and he suggested palliative care only. The Office of Aging agreed. On January 20, 2006 Mr. Moffitt began receiving additional care through Asera Hospice. On February 1,2006 I was notified by the social worker at Asera Hospice that Mr. Moffitt's condition worsened. He was using more oxygen, his color was poor and his respirations were slowing. I notified the family of this Rt change in his condition and recommended they visit. Mr. Moffitt died on February 3, 2006. Causes of death that were listed on the death certificate were respiratory failure hepatic failure, and chronic active hepatitis C. During Mr. Moffitt's hospitalization and admission to a nursing home, he was visited numerous times by Office of Aging staff. In addition, we met with nursing home staff for care plan meetings and other issues, purchased clothing and other needed items for Mr. Moffitt. Barley Director Report from Guardian of Estate Cumberland County Office of Aging Janet Paull, Aging Care Manager III \.....' . ~ "'-'J ~ '; ~ ~ On May 25,2005 Guardianship of Estate for Mr. Moffitt Moffitt was granted by Judge Edward E. Guido to the Cumberland County Office of Aging. Several months prior to this,Mr. Moffitt's nephew, who had been made Power-of- Attorney, opened a checking account for him. The deposits were Mr. Moffitt's monthly Supplemental Security Income (SSI) check. This was his only income since he did not qualify for Social Security. Monies transferred from this account ($1500.00) were used to start a guardianship account at Orrstown Bank. The Office of Aging applied for Medical Assistance for Mr. Moffitt, which began in September 2005. At that point in time his SSI benefit was reduced to $30. Additional monies were transferred from the other account, closing out that account. Early in January, 2005 prepaid funeral arrangements were made with the Egger Funeral home in Newville, where Mr. Moffitt formerly lived. Mr. Egger was willing to make the arrangements even though the available funds were $200 dollars less than the lowest priced arrangement. When it became apparent that Mr. Moffitt's death was very imminent, a check for the remainder of his account ($17.37) was sent to the funeral home.
