The Mother of the child is ~ancy Danley Sersch, currently residing at 1426~. Market Street, Camp Hill, Cumberland County, Pennsylvania 17011. The parties are divorced from each other. 2 8. The relationship of the Plaintiff to that of the child is that of Father. The Plaintiff currently resides with the following persons: NAME John N. Sersch Rhonda D. Gaudette Sersch RELATIONSHIP Self Wife 9. The relationship of the Defendant to the child is Mother. The Defendant currently resides with the following persons: NAME RELATIONSHIP Self Child Nancy Danley Sersch Jenna N. Sersch 10. Plaintiff has not participated as a party or witness, or in another capacity, in other litigation concerning the custody ofthe child in this or another court. II. The Plaintiffhas no infonnation of a custody proceeding concerning the child pending in any court of this Commonwealth. 12. Plaintiff does not know of a person not a party to the proc(~edings who has physical custody of the child or claims to have custody or visitation rights with respect to the child. 3 13. The best interest and permanent welfare of the child will be served by granting the relief requested because Plaintiff can provide a stable, loving environment for his child. Defendant is not attentive to the child and her busy and social lifestyle and does not place the child as a priority. Defendant lacks appropriate sleeping quarters for the child. The child is often cared for by others. Father can provide the child with the love, care and attention she needs and desires. 14. Each parent whose parental rights to the child have not been terminated and the person who has physical custody of the child has been named as parties to this action.
