DECREE
AND NOW, this .11a.l~day of \j\I1", L l , 2006, upon
consideration of the Petition for Settlement of Small Estate
filed in the above-captioned matter, and the exhibits attached
thereto, the relief requested in the Petition is hereby granted.
The Court is satisfied that proper notice has been given to
the parties in interest. In this regard, the Court takes
judicial notice that the Consents of Petitioner's children (the
other possible intestate heirs) are attached to the Petition.
In accordance with 20 Pa.C.S. ~3102, Members 1st Federal
Credit Union is hereby ordered to pay the proceeds of the
certificate of deposit held under account number 7117, having a
maturity date of November 29, 2006, currently titled in the name
of Dwight P. smith, to his widow, Petitioner, Rita M. Smith.
Also, Holy Spirit Hospital is hereby ordered to release to
Rita M. smith any such medical records pertaining to Dwight P.
Smith, as Rita M. smith may request.
