NOW COMES Upper Allen Townsbip, Cumberland County, Pennsylvania, by its
Townsbip Manager, Louis Fazekas, and files tbe witbin claim for cbarges assessed
Witb respect to sewer and/or water rates against above named owner or owners and
against tbe bereinafter described real estate as follows:
STATEMENT OF CLAIM
1. Tbis claim is filed by UPPER ALLEN TOWNSHIP, a first class townsbip
organized and existing in accordance witb tbe laws oftbe Commonwealtb of Pennsylvania.
2. Tbe owner or reputed owner of tbe property, or party or parties
responsible or liable, for wbicb tbis claim is filed is tbe Defendant (s).
3. Tbe antbority under wbicb tbe sewer and/or water rates were cbarged
and assessed is tbe Act of July 18, 1935, P.L. 1286. Section 1,53 P.S. ~ 2231, as
amended.

4. Tbe autbority under wbicb tbis claim is filed is tbe Act of May 16, 1923,
P.L. 207, Section 9, et seq. 53 P.S. ~7143, as amended.
5. A description of the property against whicb this claim is filed is:

Upper Allen Townsbip
Cumberland County
Mecbanicsburg, Pennsylvania 17055
Tax parcel Number: 42-28-2423-130
6. Tbis claim has not been the subject of any previous claim and judgment in
tbe Court of Common Pleas of Cumberland County for sewer/water charges.
7. Tbe total amount of tbe charges for wbicb tbis claim is filed is
Five Hundred, Forty-Nine and lI/OO dollars ($549.11) plus costs.
8. Tbe time period for wbicb tbese cbarges are filed is October 2004 to
February 28, 2006.
WHEREFORE, Claimant requests tbat a Municipal Lien be entered in its favor
and against the Defendant in the amount of Five Hundred, Forty-Nine and II/OO
dollar ($549.11) plus costs.
