Pennsylvania, by its staff attorney David J. Spotts, Esquire, and files the within claim for
charges assessed with respect to sewer and/or refuse rates against above named
owner or owners and against the herein fter described real estate as follows:

amended.

6. This claim has not been th subject of any previous claim and judgment in
the Court of Common Pleas of Cumberl nd County for sewer and/or refuse charges.
7. The total amount of the ch rges for which this claim is filed is six hundred
and forty-six and 92/100 dollars ($646,9 ), plus costs and reasonable attorney's fees.
8. The time period for which hese charges are filed is October 1, 2004 to
March 31, 2006 on Borough Sewer Ace unt(s) 55-005425.
WHEREFORE, Claimant reques s that a Municipal Lien be entered in its favor
and against the Defendant in the amou t of six hundred and forty-six and 92/100
dollars ($646,92), plus costs and reaso able attorney's fees.
